- Many patients on biologics, medications prescribed in hospital clinics, or who have specialist medications delivered direct to their homes will have these listed on the bottom of their ECS with the phrase “outwith practice” marked next to them.
- Even if the item is dated several years previously, ask the patient if they are still receiving this medication and how they receive it.
- If they are still receiving the medication it will need to be prescribed on HEPMA – suspend if you are not sure if it is clinically appropriate to continue at admission.
- Highlight all such medications to the ward pharmacist so they can inform the appropriate provider and ensure continuity of supply for the patient.
