Safe and Effective Prescribing and Use of Medicines for Patient Safety
Methadone / Buprenorphine
- Highlight these patients to the ward pharmacist.
- Our policy at admission is to prescribe and immediately suspend methadone / buprenorphine until dose and recent collection history can be independently confirmed.
- During working hours, call the community pharmacy to confirm dose, recent collection / administrations and alert them that the patient is in hospital. If the patient has missed 3 or more consecutive doses, seek advice from the Drug and Alcohol Liaison Service.
- If the patient is admitted out of hours, check Clinical Portal for dose information. Ask the day team to highlight these patients to the ward pharmacist.
